!!ダイビング(体験/ファン)に参加される方はご確認ください / Requirement to join Diving(Intro/Fun) course!!
"ダイバーメディカル/Diver Medical"を確認し下記に同意します。I read "Diver Medical" and agreed to below.

①ダイビングの後、同日中は飛行機に乗れません。
②ダイビングに適した健康状態か上記の”ダイバーメディカル/Diver Medical"を読んで確認してください。
③60歳以上の方はダイバーメディカル/DiverMedicalフォームに、医師による同意を受けて下さい。
①Must NOT take a flight on the same day after the dive.
②Please check your health condition with "Diver Medical" form above.
③If you are 60years old or above, doctor's agreement on Diver Medical Form is required.
